Ecografía versus tomografía en abdomen agudo: rendimiento diagnóstico y correlación de hallazgos imagenológicos con el diagnóstico definitivo

    Resumen

    El abdomen agudo exige decisiones diagnósticas rápidas porque múltiples enfermedades comparten manifestaciones clínicas y pueden evolucionar hacia complicaciones graves. El objetivo fue comparar el rendimiento diagnóstico de la ecografía abdominal y la tomografía computarizada, con énfasis en apendicitis, colecistitis, diverticulitis y obstrucción intestinal, y establecer su correlación con el diagnóstico definitivo. Se desarrolló una revisión sistemática cualitativa de evidencia de exactitud diagnóstica, estructurada con PRISMA 2020, principios del Cochrane Handbook y criterios de riesgo de sesgo de QUADAS-2, sin efectuar un metaanálisis de novo. La tomografía mostró mayor rendimiento en apendicitis, con sensibilidad de 97,2 % y especificidad de 95,6 %, frente a 82,1 % y 85,9 % para ecografía; en colecistitis las diferencias fueron menores, mientras que en diverticulitis y obstrucción la ecografía alcanzó valores elevados para detección, pero la tomografía aportó mayor capacidad para caracterizar extensión, etiología y complicaciones. Se concluyó que ambas modalidades son complementarias, con una estrategia escalonada que favorece ecografía inicial en escenarios seleccionados y tomografía ante incertidumbre, diagnóstico diferencial amplio o sospecha de enfermedad complicada.
